FEES AND CO-PAYS

As a 501(c) Non-Profit, MAB services are subsidized through ABS production revenue, United Way, and private donation. Therefore clients are not charged the $150 per hour fee for direct service hours provided. Client donations of any amount are accepted and greatly appreciated.

Claims will be submitted to primary and secondary insurances for clients receiving a low vision exam or training by our agency optometrist or occupational therapist. At the time of the exam and/or low vision evaluation, a $35 fee will be due.

Beginning July 1st, 2011 the low vision exam fee will be $40. Fees are based on a sliding-scale.

In an effort to provide ongoing transportation service to clients, beginning July 1st, 2011 a $5 fee will be implemented for each scheduled trip. This fee will help offset the $48 cost of each trip. Fees are based on a sliding-scale.
