FEES AND CO-PAYS

If you have Medicare Part B, as part of a Medicare Demonstration Project, the Centers for Medicare and Medicaid Services allow MAB to charge up to 12 hours annually for services provided by an occupational therapist or certified low vision therapist, orientation and mobility specialist and vision rehabilitation therapist. MAB will also submit to secondary insurance. You will not be charged for any unpaid amount from our Medicare submission. If the patient has Medicare Part B and chooses not to permit MAB to submit for services rendered, they will become responsible for all fees which are currently set at $300 per direct service hour.

If you receive a low vision exam or training by our agency optometrist, we will forward the fee information to your primary and secondary insurance for payment. At the time of the exam and/or low vision evaluation, a $35 fee will be due.
