CASE MANAGEMENT

Case management services are offered to each client on an individualized basis. Case management is an ongoing service that is provided to meet the changing needs in a client's life as he/she adjusts to vision loss. Each client will receive a case manager based on the client's zip code area.

The case manager and client will meet initially to talk about the client's needs in relation to services offered by MAB and other local agencies. From that point, the client and case manager will establish goals related to the client's needs. The case manager will make referrals within MAB as well as community agencies to get the client connected with appropriate resources to help meet those goals.
